AMSTERDAM – Every week a simple operation in a Dutch hospital goes wrong. A specialist amputates the wrong toe from a diabetes patient, for example.
This is the painful result of poor communication between health care providers, inadequate record keeping and the failure to hand over patient information.
Inspector general Gerrit van der Wal of the Healthcare Inspectorate (IGZ) cited this example after a report into "shortcomings in the preoperative process" was published on Tuesday.
